Symptoms almost always begin years before periods stop, in perimenopause, and that is where many women do not recognise themselves in what they feel: cycles that change rhythm, hot flushes, night sweats, broken sleep, irritability, difficulty concentrating, joint pain, vaginal dryness or loss of desire. This consultation is for that stage and for afterwards too, for anyone who wants to review their current treatment or understand what changes in bone and cardiovascular risk.

The doctor maps your symptoms and their impact on daily life, reviews your history and tests, and orders those that are useful. Hormone tests are not always needed to reach the diagnosis, and they will explain why. They then go through the options with you, hormonal and non-hormonal, with the benefits and risks of each given your history. The decision is shared and the indication is clinical.

This is an endocrinology consultation, focused on the hormonal, bone and cardiometabolic side. It does not include a gynaecological examination, cervical screening or pelvic ultrasound. When any of those is needed, the doctor will tell you and refer you to an in-person gynaecology appointment.

Any bleeding more than a year after your periods stopped needs to be assessed, even if it is light and passes. Do not wait for your next appointment.

How to prepare

Note the date of your last periods or how long ago they stopped, and keep a simple record of your symptoms over recent weeks: which ones, how often, and what bothers you most. Gather recent tests and your bone density scan, if you have had one, along with your medication and supplement list. It helps to know whether there is a family history of breast cancer, osteoporosis or early cardiovascular disease: it weighs on the treatment decision.
